Management of Advanced Glaucoma

On Ophthalmology off the Grid, Nathan M. Radcliffe, MD, joins hosts Blake Williamson, MD, MPH, and Gary Wörtz, MD, to share his experiences managing complex patients with advanced-stage glaucoma. He shares a specific case of an 83-year-old man with a cataract and exfoliation glaucoma with an IOP of 56 mm Hg. Dr. Radcliffe walks through how he managed complications he encountered during the combined cataract and glaucoma surgery, including vitreous prolapse and suprachoroidal hemorrhage. The surgeons also discuss how to educate and counsel patients with advanced-stage disease to help them understand realistic surgical outcomes.


Xen Gel Stent Implantation Pearls

On Glaucoma Case Files, Manjool Shah, MD, describes a patient with glaucoma who had previously undergone successful tube shunt surgery in her right eye and was now losing vision in her left eye. Dr. Shah explains why he decided the Xen Gel Stent (AbbVie) was the most appropriate intervention for this patient and why a tube shunt was not an option due to the patient’s scleral integrity. Joseph F. Panarelli, MD, and Arsham Sheybani, MD, share additional insights into patient selection and how to determine appropriate treatment strategies. Dr. Shah further explains his preferred placement technique for Xen Gel Stent implantation.
