Lensectomy Gone Awry

Arsham Sheybani, MD, joins the Ophthalmology off the Grid podcast to continue this season’s theme of surgical horror stories. He shares a case example with hosts Blake Williamson, MD, MPH, and Gary Wörtz, MD, in which he performed a lensectomy in the eye of a 60-year-old woman who presented with angle-closure glaucoma and very high IOP. Following routine phacoemulsification, the anterior chamber became shallow, eventually resulting in suprachoroidal hemorrhage. Dr. Sheybani also shares advice for communicating with patients about difficult, unfortunate outcomes.

Hypotony Challenge: Choroidal Effusion Drainage and Compression Suture

On Inside the Wills OR, Reza Razeghinejad, MD, presents a case of a patient who had a history of trabeculectomy and hypotony with a large overfiltering bleb and choroidal effusion. He demonstrates his technique for draining the choroidal effusion and using compression sutures. The surgical video shows Dr. Razeghinejad creating a full-thickness triangular scleral flap to allow effusion drainage through the scleral window.

A Novel Physics-Based Device for Lowering IOP

On Clinical Minute, Thomas Samuelson, MD, joins Neda Shamie, MD, to discuss a prospective, assessor-masked, randomized controlled trial that evaluated the safety and effectiveness of a multipressure dial system (Balance Ophthalmics) for IOP reduction. What were the results?
